LSNA Health Outreach Team Provides Community Resources

Times are hard. We can all use some help. Let us help you.
You may qualify for Food Stamps, now called SNAP (Supplemental Nutritional Assistance Program), and don’t know it.  Whether you are working, have been laid off, have children or don’t have children you may be able to get assistance to buy food.
 Since January 2009, the Health Outreach Team at the Logan Square Neighborhood Association has helped 250 families apply for SNAP. You can visit our office and be assisted at no cost. Recently we have seen many people who are unemployed and have never used public benefits (programs funded by the government). They don’t know what resources are available to them.
 Do you qualify?  Chances are good that families or individuals may qualify if they are:
low income,
children receive free lunch at school,
someone is a legal permanent resident for 5 years or more
children were born in the United States regardless of their parents’ legal status. 
 
 The Health Outreach Team at LSNA is bilingual. You can visit us with or without an appointment Monday through Friday. Our team has been doing this work for over 9 years. We can also link the uninsured and underinsured with health resources.
